Pharmacist - Fort Wayne, Indiana

Job Summary:

We are seeking a dedicated and detail-oriented Pharmacist to oversee daily pharmacy operations, ensure compliance with all regulatory standards, and support patient-centered care. This role includes managing medication therapies, supporting the 340B program, and collaborating with healthcare providers to optimize patient outcomes.

Key Responsibilities

• Review and evaluate medication histories, ensuring appropriate drug selection and patient compliance
• Provide patient counseling on medication use and identify barriers to adherence
• Verify medication orders for accuracy and appropriateness
• Perform medication reconciliation and assess therapy based on patient-specific data
• Identify, prevent, and resolve medication-related issues
• Collaborate with healthcare providers to recommend evidence-based therapies
• Monitor for contraindications, duplicate therapies, and cost-effective treatment options
• Respond to medication-related inquiries from patients, providers, and staff
• Administer immunizations as appropriate
• Ensure proper handling and dispensing of controlled substances
• Maintain pharmacy inventory and order supplies
• Support and manage participation in the 340B drug pricing program
• Use EHR and pharmacy systems to manage tasks efficiently
• Communicate with payers to resolve medication access issues
• Ensure compliance with all state and federal pharmacy regulations
• Participate in quality improvement and patient safety initiatives

Qualifications:

• Education: Doctor of Pharmacy (PharmD) preferred or Bachelor’s in Pharmacy
• At least 1 year of pharmacy experience preferred
• Experience with 340B program preferred
• Active pharmacist license
• Immunization certification required

Physical & Work Environment Requirements:

• Ability to stand, walk, bend, and lift up to 50 lbs
• Comfortable working in a clinical pharmacy setting
• Exposure to communicable diseases and hazardous substances
• Ability to work flexible hours, including evenings
• Frequent interaction with patients, staff, and healthcare providers
