As a Clinical Liaison , you are the bridge between patients, families, and life-changing rehabilitation services.
You’ll combine your clinical expertise + relationship-building skills to grow our program, guide patients to the right level of care, and make a meaningful difference in your community.
What You’ll Do
✨ Build strong referral relationships with hospitals, physicians, case managers, and community partners
✨ Educate referral sources on the power of inpatient rehabilitation
✨ Conduct clinical assessments to identify appropriate admissions
✨ Review patient charts, therapy evaluations, and medical history
✨ Navigate and remove barriers to admission
✨ Complete detailed Pre-Admission Screens in compliance with regulations
✨ Coordinate meetings, in-services, and outreach events
✨ Provide ongoing updates to physicians, payers, and care teams
✨ Deliver exceptional follow-up and service to referral partners
What We’re Looking For
✔️ Clinical background required (RN, PT, OT, SLP, LVN, PTA, COTA)
✔️ Bachelor’s degree in healthcare, business, or related field preferred
✔️ Strong communicator with relationship-building skills
✔️ Comfortable working in the field—meeting referral sources face-to-face
✔️ Valid driver’s license + reliable transportation
